6.1 Feedback: why?

The aim of incident and accident analysis is to avoid (re)producing a catastrophic scenario, not to find a culprit. Accident databases can be used to identify trends: what is the profile of accident victims, what type of laser and wavelengths are involved, which operations are accident-prone...?
