Accidents, incidents, crises: severe events occur regularly within industrial organizations. Accidents are not solely attributable to human error (the "human factor") and/or technical failure. Generally, accidents announce" themselves through a certain number of deep dysfunctions in the organization which increase all along the incubation period preceeding the catastrophy. The organizational analysis is based upon the in-depth examination, within the thickness of the context, of the multiple "vertical" interactions (between management, expertise and fielsd staff) and "horizontal" ones (between the various entities which constitute the organization).
