Those in charge of feedback systems generally consider the completeness of data collection to be the main problem.
With the exception of the most serious events, which do not go unnoticed, knowledge of events depends on the goodwill of those involved in the field. For all events, the quality of analysis depends on the willingness of those involved to share information and tell the truth. There can be no quality feedback without the motivation of those involved to bring it to life! You can't simply declare feedback to be effective; it has to be in the interest of everyone involved.
